Meet Our Team
Dr. John W. Mann, III
MD, FACP
A Bristol native, Dr. John Mann began his healthcare career in EMS before serving as an Army medic with the 10th Mountain Light Infantry. He later practiced as a physician assistant before attending medical school and completing his Internal Medicine residency. Board certified in Internal Medicine and a Fellow of the American College of Physicians, Dr. Mann founded Club Level Healthcare to provide the patient-centered care he envisioned when he first entered medicine.
Mike F. Baines
MHS, PA-C
Bristol native Mike Baines brings decades of healthcare experience to Club Level Healthcare. His career began with the Bristol Life Saving Crew, where he became a paramedic before continuing his education at ETSU and earning his master's degree as a physician assistant. His experience in both emergency medicine and personalized primary care gives Mike a broad perspective on meeting patients' needs while building lasting relationships.
Haley Pender
Office Manager
Haley helps make the Club Level experience personal from the moment you contact the practice. As Office Manager, she's a familiar point of contact for patients, helping with scheduling, questions and the day-to-day details that make accessing your healthcare team simple and comfortable.

*Our practice specializes in adult primary care while also offering convenient care for children when acute medical needs arise. We encourage families to maintain a pediatrician for their child's ongoing wellness and routine pediatric care, while we remain available when your child needs prompt attention for an acute illness or concern.

Concierge medicine works differently from traditional primary care. Here are answers to some of the questions we hear most often.
What is concierge medicine?
Concierge medicine is a membership-based approach to primary care that allows providers to care for a smaller number of patients. Our approach at Club Level Healthcare means more time for appointments, greater access to your healthcare team, and care built around an ongoing relationship with you.
Is concierge medicine insurance?
No. Your Club Level Healthcare membership is not health insurance. Membership provides enhanced access to our practice and the services included in your membership. We recommend maintaining health insurance for services outside our practice, including specialists, hospital care, imaging, emergency care, and other services not provided by Club Level Healthcare.
Do you accept insurance?
Club Level Healthcare operates primarily through an annual membership rather than traditional insurance-based primary care. Your membership covers the care and access included in your plan, while health insurance may still be used for services provided outside our practice, such as specialists, imaging, hospital care, and prescriptions.
What does my membership include?
Membership includes extended, unhurried appointments, direct access to your healthcare team, same-day or next-day appointments when available, virtual visits and flexible scheduling, preventive and wellness-focused care, and personalized care coordination.
There are no enrollment fees.
How quickly can I get an appointment?
Because Club Level Healthcare limits the size of the practice, we can offer greater scheduling flexibility than many traditional primary care practices. Same-day or next-day appointments are often available when needed.
What if an urgent medical need arises after hours?
We understand that medical concerns don't always happen during regular office hours. As part of our commitment to personalized care, you are welcome to call or text us after hours with non-emergency medical concerns. We do our best to respond within a couple of hours, depending on the time your message is received and our availability.
Note: Our practice does not provide emergency medical services. If you are experiencing a medical emergency or symptoms that may be life-threatening, please call 911 or go to the nearest emergency room.
Your Health and peace of mind matter to us, and we will do our best to help guide you toward the appropriate care when you need it.
Can I have virtual appointments?
Yes. Virtual visits are available when appropriate, giving you a convenient way to connect directly with your healthcare team without always needing to come into the office.
Do you see children?
Club Level Healthcare specializes in adult primary care while also offering convenient care for children when acute medical needs arise. We encourage families to maintain a pediatrician for their child’s ongoing wellness and routine pediatric care, while we remain available when your child needs prompt attention for an acute illness or concern.
Note: For children younger than 10, appointments are considered on a case-by-case basis at the provider's discretion.
