Membership in Club Level Healthcare includes direct access to your providers by phone, text and email. There is no one between you and your provider. Calls are made directly to your providers personal cell phone.

All office visits are included with no additional fee. Most visits can be arranged within 24 hours and many can be arranged for the same day. Many appointments occur outside normal office hours including early morning, evening and weekends. Home visits are available at the discretion of the provider.

One set of labs are including with your yearly membership and are usually performed at the time of your physical. If there is a need for further labs during the year you may take an order to a commercial lab or you can be billed from the office at our discounted price usually lower than your insurance co pay.
